Forms

The following forms must be completed by the employer:

• Active Teacher Death
• Eligible Employee Designation Request
• Substitute Service Purchase Application (The member will fill out part of the form and then submit it to each applicable school division.)
  • If you need more space on the Substitute Service Purchase Application, fill out the Substitute Service and Earnings Information form and submit it to TRAF with the original application form.
